So Long But Not Goodbye

When I was nominated for the position of President for a 4th term in 2023, I told the Board that I would not accept a nomination in 2024, but not for the reasons you may think. I had accepted a new position with a national hospital system, and although it is a fully remote position, there are requirements for me to be in attendance at trials, mediations, depositions and other events, meaning that I would need to travel at times and my schedule can vary at a moment's notice which could interfere with my ability to lead board meetings and other obligations that arise out of being the IPA's President. On a personal level, my husband's parents have some health challenges and now that our children have become adults, we made the decision to move in with them so they can remain in their home rather than moving them into an assisted living facility.

Important Notices and Useful Information

The Illinois Supreme Court Commission on Access to Justice is excited to announce the republishing of five statewide standardized court forms in our new redesign format.
  1. The Appearance and Jury Request form set includes a Jury Request as a distinct form following feedback we received from court stakeholders.
  2. The https://www.illinoiscourts.gov/documents-and-forms/approved-forms/circuit-forms/answer-response form set includes distinct Counterclaims and Affirmative Defense forms that can be filed at the same time as the Answer. They were designed as separate forms to accommodate the e-filing and associated fees processes. They should be filed separate but at the same time as the Answer, and legally they will be considered part of the Answer.
  3. The Motion was updated in the redesign format.
  4. The Proof of Delivery form set was updated in the redesign format.
  5. The Limited Scope form set was updated in the redesign format.
If you are interested in learning more about these forms and the redesign project, here is a short video going over these five new redesigned statewide standardized forms. Please note that many forms will remain in the older format while we make the transition into our new design. All statewide forms must be accepted by courts whether they are in the old or redesigned format.

As a reminder, we will not immediately announce every minor update, so please make sure that you are directing court users to the forms via our website. Do not link directly to saved PDFs of the forms because you may miss an updated version. For your convenience, we have a complete list of our currently published forms and their respective form numbers.
